Inpatient Rehab Documentation Coordinator

About the role

The Inpatient Rehab Documentation Coordinator ensures that the medical record accurately reflects patient complexity and resource utilization. Reporting to the Rehab Quality/PPS Manager, the role bridges clinical care, compliance, and reimbursement while representing the RN voice at weekly interdisciplinary team meetings.

Key responsibilities

• Review and improve clinical documentation to align with IRF-PAI requirements.
• Lead discussions during interdisciplinary team conferences with physicians, therapists, and case managers.
• Conduct direct clinical interviews and record reviews to verify patient status.
• Maintain compliance with coding standards and functional scoring.

Required profile

• Active Registered Nurse (RN) license.
• Minimum 3 years of clinical experience in an Inpatient Rehabilitation Facility or Acute Care setting.
• Proficiency in ICD-10-CM coding and Section GG functional scoring.
• Strong ability to communicate confidently with multidisciplinary teams.
• High attention to detail to identify clinical inconsistencies.
