Registered Nurse – Patient Support / Care Coach

About the role

The Registered Nurse will act as a Patient Support and Care Coach, providing compassionate guidance to help patients manage their condition and adhere to treatment. Working both in‑person and by telephone, the nurse builds rapport, educates patients and families, and coordinates communication between patients, physicians, and payers.

Key responsibilities

• Introduce physicians to the Patient Care Consultant (PCC) role and obtain consent for patient contact.
• Secure signed consent forms from physicians and patients for education, self‑injection training, and health data collection.
• Capture and maintain accurate patient details in the program’s CRM system after face‑to‑face and telephone interactions.
• Protect patient data privacy and ensure confidentiality at all times.
• Provide regular updates to each physician on patient status, including condition, compliance, side effects, and emotional wellbeing.
• Visit physicians as scheduled to present patient progress reports.
• Educate patients, parents, and caregivers about the disease, treatment storage, administration, and self‑injection techniques.
• Assess and address psychological or behavioural challenges that may affect treatment adherence.
• Manage and report adverse events, product quality issues, and safety risks according to Pharmacovigilance SOPs.
• Respond to patient queries and follow up on questions regularly.

Required profile

• Qualified Registered Nurse or equivalent healthcare professional.
• Experience in patient education, support, or care coordination.
• Strong communication skills for interacting with patients, families, physicians, and payers.
• Ability to maintain strict confidentiality and data‑privacy standards.
• Willingness to travel for physician visits as required by the project plan.

Required skills

• CRM system usage for data entry and tracking.
• Knowledge of Pharmacovigilance SOPs and adverse‑event reporting.
