Inpatient Rehab Documentation Coordinator

About the role

The Inpatient Rehab Documentation Coordinator supports accurate clinical documentation within an inpatient rehabilitation facility. Reporting to the Rehab Quality/PPS Manager, the role ensures that patient records reflect true complexity and resource use, linking care delivery with compliance and reimbursement processes.

Key responsibilities

• Review and validate inpatient rehabilitation records to ensure alignment with IRF‑PAI and Section GG functional scoring.
• Serve as the RN representative on weekly Interdisciplinary Team conferences, leading discussions with physicians, therapists, and case managers.
• Identify and resolve clinical inconsistencies across disciplines through chart audits and direct patient interviews.
• Maintain up‑to‑date knowledge of ICD‑10‑CM coding guidelines and apply them to documentation improvement initiatives.
• Collaborate with the Rehab Quality team to monitor compliance and support accurate reimbursement.

Required profile

• Active Registered Nurse (RN) license.
• Minimum three years of clinical experience in an inpatient rehabilitation or acute care setting.
• Strong analytical mindset with high attention to detail.
• Excellent communication skills, comfortable presenting to multidisciplinary teams.

Required skills

• ICD‑10‑CM coding proficiency.
• Section GG functional scoring expertise.
