Inpatient Rehab Documentation Coordinator

About the role

The Inpatient Rehab Documentation Coordinator ensures that the medical record accurately reflects patient complexity and resource utilization in an inpatient rehabilitation setting. Reporting to the Rehab Quality/PPS Manager, the role bridges clinical care, compliance, and reimbursement while representing the RN voice at interdisciplinary team meetings.

Key responsibilities

• Review and improve clinical documentation to align with IRF-PAI and reimbursement guidelines.
• Serve as the RN representative at weekly Interdisciplinary Team (IDT) conferences, leading discussions with physicians, therapists, and case managers.
• Conduct direct clinical interviews and record reviews to verify patient status and functional scores.
• Identify and resolve documentation inconsistencies across multiple disciplines.
• Maintain accurate coding using ICD-10-CM and Section GG functional scoring.

Required profile

• Active Registered Nurse (RN) license.
• Minimum 3 years of clinical experience in an Inpatient Rehab Facility (IRF) or acute‑care setting.
• Strong analytical attention to detail.
• Confident communication skills for leading interdisciplinary discussions.

Required skills

• Proficiency in ICD‑10‑CM coding.
• Knowledge of Section GG functional scoring.
• Experience with Clinical Documentation Improvement (CDI) processes.
