Registered Nurse – Patient Support & Care Coach

About the role

The Registered Nurse will act as a Patient Support Staff and Care Coach, providing compassionate guidance to help patients manage their condition and adhere to treatment plans. Working closely with physicians, patients, and third‑party payers, the nurse ensures continuous education, motivation, and follow‑up.

Key responsibilities

• Introduce the Patient Care Coach role to physicians and obtain necessary consent forms.
• Conduct regular in‑person and telephone contacts with patients, building rapport and addressing personal challenges.
• Accurately capture patient interactions and assessments in the programme CRM system.
• Maintain 100% data privacy and confidentiality.
• Provide physicians with regular updates on patient status, including condition, compliance, side effects, and emotional wellbeing.
• Educate patients, families, and caregivers on disease, medication storage, self‑injection, and side‑effect reporting.
• Identify and report adverse events or safety risks according to pharmacovigilance SOPs.
• Support patients in overcoming psychological or behavioural barriers to treatment.

Required profile

• Qualified Registered Nurse with a valid nursing license.
• Experience in patient education, chronic disease management, or care coordination.
• Strong communication skills to interact with physicians, patients, and payers.
• Ability to handle confidential health information securely.

Required skills

• Proficiency in using a CRM system for data entry and reporting.
