Inpatient Rehab Documentation Coordinator

About the role

The Inpatient Rehab Documentation Coordinator ensures that the medical record accurately reflects patient complexity, resources, and outcomes in an inpatient rehabilitation setting. Reporting to the Rehab Quality/PPS Manager, the role bridges clinical care, compliance, and reimbursement while serving as the RN representative on weekly interdisciplinary team meetings.

Key responsibilities

• Review and improve clinical documentation to align with IRF‑PAI requirements.
• Conduct direct clinical interviews and record reviews to verify patient status.
• Represent the registered nurse at interdisciplinary team conferences, facilitating discussions among physicians, therapists, and case managers.
• Maintain compliance with coding standards and support accurate reimbursement.
• Identify and resolve documentation inconsistencies across multiple disciplines.

Required profile

• Active Registered Nurse (RN) license.
• Minimum 3 years of clinical experience in an inpatient rehabilitation facility or acute care setting.
• Proficiency in ICD‑10‑CM coding and Section GG functional scoring.
• Strong analytical attention to detail.
• Ability to confidently lead interdisciplinary discussions.
